Working Hours: 37.5 hours

We're looking for bright, hardworking individuals to join our established service in Dartford, Gravesham, and Swanley. It's a great opportunity to support adults who need extra help for all sorts of reasons to consider what will best improve their health and wellbeing, then make it happen by supporting them to navigate and access health, social care and community services.

It's a varied role, and you'll spend time visiting people in their own homes or in community settings, speaking to individuals and families. You'll also organise and deliver online and face-to-face group activities for adult Carers, bringing them together to experience peer support and take a break from their caring roles.

Navigators bring their own experience to the role. You might have worked in a formal setting, completing assessments and support plans, or directly providing social care or health services.

Perhaps you have worked or volunteered in a community setting or are looking to give something back to your own local community. Or maybe you have personal experience of accessing services for yourself, a relative or friend.

You'll get to know local services, build networks and work in partnership with other agencies, and will work alongside colleagues in a friendly team so you will feel fully supported. This is a full-time role, and a car driver is essential.
