Social Prescribing Care Coordinator

As a Social Prescribing Care Coordinator specialising in mental health and based in GP practices, you will provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals.


You will work with identified patients to improve patient-centred care and relationships with health professionals, providing support to them and their carers to ensure their changing needs are met. You will coordinate support and services, ensuring patients do not fall into any gaps while waiting for ongoing support, as well as coordinating other appropriate support via community or social interventions that will aid recovery. By supporting individuals to develop their knowledge, skills and confidence, patients will have greater engagement in their health and wellbeing.


In this role, you will provide coordination and navigation for people and their Carers across Health and Care Services, working closely alongside Social Prescribing Link Coordinators and other Primary Care roles. You will provide good quality information to enable patients to make choices and decisions about their care and support needs, bringing this all together in a personalised support plan.


We are looking for an organised, self-motivated and confident individual to join our established team. The position is available in Rainham, and applicants must have experience of working with vulnerable adults, a background in medical or social care and experience of delivering mental health support to clients or groups. It is essential that you are confident in your ability to communicate at all levels across both the health and social care sectors.


You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.

Salary:             £19,000-22,000 per annum depending on experience
