Imago can provide free training for your surgery and support
you to implement a Young Carers Strategy, which will show your
compliance with the Care Act 2015 / Children’s and Families
Act 2015; demonstrating good practice when CQC Inspected
 

  • Relevant for Practice Managers, GP’s and  Support Staff
  • 2 ½ Hours’ worth of training (can be condensed to an hour to suit the needs of the practice)
  • For up to 15 staff members (minimum of 3)
  • Free Training for a limited time

Social prescribing project

New project launching October 2018 

Imago working in partnership with NHS DGSS Clinical Commissioning Group (DGSS CCG), are pleased to announce that we are one of twenty-three organisations across England that have been awarded funding to deliver an innovative social prescribing project to improve patients quality of life, health and wellbeing. 

The proposed Imago model will expand on an existing scheme to align with the new sustainability and transformation partnership model for local care across Dartford, Gravesham and Swanley.

The scheme covers both the Integrated Discharge Team at Darent Valley Hospital and the Downs Way Medical Practice, and the expansion will reach 36 GP practices and identify patients with multiple conditions and complex needs and provide additional support.  A team of volunteer peer supporters in each local care team will be recruited and supported to add value to the paid social prescribing link worker roles.
