The Imago model will expand on an existing scheme to align with the new sustainability and transformation partnership model for local care across Dartford, Gravesham and Swanley.
The scheme covers both the Integrated Discharge Team at Darent Valley Hospital and the Downs Way Medical Practice, and the expansion will reach 36 GP practices and identify patients with multiple conditions and complex needs and provide additional support. A team of volunteer peer supporters in each local care team will be recruited and supported to add value to the paid social prescribing link worker roles.
Social Prescribing Volunteers
Imago is delighted to be able to provide a new Social Prescribing service in the Dartford, Gravesham and Swanley areas. The Social Prescribing service provides free short-term intervention and support to adults with complex health and social care needs living in the community. The support develops the client’s confidence, self-esteem and resilience, with a longer-term aim of reducing dependency on GP and health services. Social Prescribers work with individuals to enable them to look after their own health and wellbeing, to connect with others and to stay independent.
There are opportunities to volunteer as a peer mentor alongside the Social Prescriber staff team to support the client to progress and achieve their desired outcome; this may take the form of the volunteer visiting the client at home or meeting them in a community-based setting. You could provide help in a number of ways such as helping with household bills or budgeting; having a chat over a cup of tea; introducing people to a social activity or setting up and running a befriending group.
