The Wellbeing Navigator role is part of the wider community services re-design programme to improve support to patients by assisting them to better navigate through health, social care and voluntary sector support services. 

The Wellbeing Navigators offer free impartial information, advice, and guidance to help people get the assistance they require. They carry out holistic assessments with individuals, their families and carers, assisting them to find the right opportunities locally. 

 

 

How the Wellbeing Navigators Can Help

Wellbeing Navigators work across Medway offering a personalised approach, enabling people to remain in their own home, increase their ability to take control and self-manage. Help is offered through home visits, face to face appointments, telephone or email.

 

We will link people with appropriate local services and organisations who can provide ongoing help such as:

  • Home Safety
  • Financial Support
  • Housing
  • Aids and adaptations
  • Sensory Services
  • Domestic Support
  • Shopping
  • Domiciliary
  • Care Social Activities
  • Form Filling 
  • Transport
  • Gardening
  • Handyman Services

How to Access the Service

Referrals to this service can be made via GP Practices, Medway Community Health, Voluntary Sector, Social Care Professionals or you can contact the service yourself.
