City of London Care Navigator contract

Imago awarded Care Navigator contract

We are delighted to announce that the City of London Corporation has awarded Imago the Care Navigator contract. Our service delivery will commence on 3rd November 2025.

The hospital Care Navigator service will support the safe and timely discharge of residents in the City of London’s Square Mile throughout their transition from hospital to home. We will support individuals to access local community, health and social care services to enhance wellbeing, resilience, and confidence in living independently after discharge.

This service will complement our existing delivery of the City of London Carer Support service.

We look forward to strengthening the strong partnerships already in place within the City to provide coordinated, person-centred support for residents.

The Hospital Care Navigation Service is commissioned by the City of London Corporation.
