Social Prescribing Day is an annual celebration highlighting the impact of social prescribing on people and communities.

Social prescribing can help you have more control over your own health and wellbeing. Imago social prescribers will listen to you to understand your situation and support you to feel healthy. We offer social prescribing support in GP surgeries in Dartford, Gravesham, Swanley, Swale, Medway and Sheppey.

We work closely with your GP practice providing practical and emotional support and connecting you to people and activities that might help you feel better.
