What support will I receive?
We provide information and guidance, one-to-one support via a named coordinator, access to a structured 8-week Empower and Thrive programme and signposting to community, health and social care services – helping you to live independently and enjoy a more connected life.
We can help with:

Personalised 1:1 support
targeted, time-limited support at home or in community settings for individuals with more complex needs, including those 18+

Access to our Empower and Thrive programme
weekly sessions designed to promote health and wellbeing, reduce loneliness and increase social connection.

Supporting you in the best way
access the service through our hub by phone, email or online to receive tailored information, advice and signposting.

Assessing your needs
we will listen to your needs and identify the types of help and support that would benefit you, including access to community groups and activities.
Register for support
Every adult registered with our service has access to our 8-week Empower and Thrive programme.
These weekly two-hour small group sessions, held in local community venues, are designed to help you improve your day-to-day life by boosting your physical and emotional health, building confidence and resilience and strengthening social connections.
Sessions are led by Imago wellbeing coordinators and external facilitators, including health practitioners and nutritionists.
Sessions include:
Understanding health and wellbeing
Supporting you to understand changes related to ageing, with guidance on joint health, posture and muscle maintenance and gentle mobility sessions with chair-based options.
Healthy eating and staying active
Including simple, realistic nutrition guidance, understanding labels and portion sizes and everyday activity, balance and mobility.
Emotional wellbeing and resilience
Discussing life transitions, identifying coping strategies, mindfulness and stress awareness.

Building confidence
The power of social connection, support networks, tips and tools for sustaining wellbeing and maintaining positive changes.
